2017 INFORMATION FOR FINANCIAL ASSISTANCE

FINANCIAL ASSISTANT APPLICANTS MUST APPLY IN PERSON AT THE SYFL OFFICE ONLY.

FINANCIAL ASSISTANCE WILL NOT BE GRANTED DURING THE LIVE REGISTRATION EVENT OR FOR ONLINE APPLICANTS. ANY PERSON THAT ATTENDS AN ONLINE OR LIVE REGISTRATION TO SIGN UP THEIR PLAYER, WILL BE DENIED FINANCIAL ASSISTANCE.

Any returning player that did not return equipment or return a volunteer form from 2016 will be denied Financial Assistance.

All Financial Assistance applications and registrations will take place on Mondays between 6pm – 7pm beginning on March 20th at the SYFL office located at 5401A Longley ln #19, 89511

COACHES/PARENTS/LEAGUE REPRESENTATIVE MAY NOT APPLY ON BEHALF OF OTHERS.

NON-REFUNDABLE PAYMENT – $100 PER CHILD. No Sibling Discounts.

You must include income for ALL ADULTS IN THE HOUSE, whether or not related to you and whether or not legally obligated to provide any support. Failing to report all income will be deemed fraud and result in revocation of any assistance granted and may result in child(ren) being expelled from the league.

THE FOLLOWING, ALONG WITH A COPY OF EACH, MUST BE SUBMITTED WITH THIS APPLICATION, ANY INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED. *IF YOU DO NOT BRING COPIES, THERE WILL BE A $2 COPY FEE FOR USE OF THE COPY MACHINE AT THE SYFL OFFICE.

2016 Completed Volunteer form for any returning FA participants Proof of Residency Driver’s License, Identification Card or Passport for adult applying for scholarship. Birth certificate for every child on application. Proof of income (2 FORMS) for all adults in household. Please provide TWO DOCUMENTS of the following per adult: 2015 W-2 2015 Tax Return 30 Days of Payroll Checks Current Unemployment Verification Letter Current Disability Verification Letter Current Letter on Verification from Medicaid/TANF/SNAP (A copy of the card WILL NOT work)

In exchange receiving financial assistance to participate in the SYFL this year, parents must commit to providing no less than ten (10) hours of volunteer service to the League for the 2017 year, PER CHILD. Volunteer forms will be provided at time of application approval. All approvals/denials will be dispersed immediately at time of application.

We are currently looking to fill our Executive Treasurer position and 3 positions on our Finance committee.

Executive Treasurer – oversees general funds and bills of the SYFL. Ensures all quickbooks are current, works on budgets and income/expense reports for the league. Sits on the Executive Board as a voting member and reports the league financial standings to the full board at quarterly meetings.

Football Treasurer – oversees all football team accounts, will meet to collect deposits and check requests, and turn all documentation and reports into the Executive Treasurer for accounting.

Cheer Treasurer – oversees all cheer team accounts, will meet to collect deposits and check requests, turn all documentation and reports into the executive treasure for accounting.

General Treasurer – assists with checking out cash boxes for games, office hours for collecting any deposits, assists with data entry as needed, or back up for other treasurer positions.

We will have an extensive interview process for any applicants.
