REMINDER - Financial Assistance Registration 4/18

Any parents that are in need of Financial Assistance will need to come in on Wednesday between 6-7pm. All of the information below is required and parents will be turned away if they do not have all of the documentation. Please make sure your coaches understand that.

COACHES/PARENTS/LEAGUE REPRESENTATIVE MAY NOT APPLY ON BEHALF OF OTHERS.

NON-REFUNDABLE PAYMENT – $100 PER CHILD. No Sibling Discounts, PROMO CODE DOES NOT APPLY.

You must include income for ALL ADULTS IN THE HOUSE, whether or not related to you and whether or not legally obligated to provide any support. Failing to report all income will be deemed fraud and result in revocation of any assistance granted and may result in child(ren) being expelled from the league.

THE FOLLOWING, ALONG WITH A COPY OF EACH, MUST BE SUBMITTED WITH THIS APPLICATION, ANY INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED. *IF YOU DO NOT BRING COPIES, THERE WILL BE A $2 COPY FEE FOR USE OF THE COPY MACHINE AT THE SYFL OFFICE.

2017 Completed Volunteer form for any returning FA participants Proof of Residency Driver’s License, Identification Card or Passport for adult applying for scholarship. Birth certificate for every child on application. Proof of income (2 FORMS) for all adults in household. Please provide TWO DOCUMENTS of the following per adult: 2017 W-2 2017 Tax Return 30 Days of Payroll Checks Current Unemployment Verification Letter Current Disability Verification Letter Current Letter on Verification from Medicaid/TANF/SNAP (A copy of the card WILL NOT work)
